Three physical locations for in person appointments
I see patients in Crown Point, Vancouver, and Salem.
Crown point is the newest location, in the Chase Bank building on the Square in Crown Point. I recommend parking in the lot on the side of the building opposite the Square. My phone numbers are all the same, and if we see each other over Doxy that address is the same as well. Please call the Salem office number for appointments, billing, insurance questions, rescheduling, and anything else.
